Ospitek Pre-Admission Health Questionnaire
Complete your medical history online—here’s how.
To ensure you receive the best care possible, Polaris Surgery Center offers an easy, online questionnaire for you to complete at your convenience.
When you use Ospitek, you avoid long phone interviews and handwritten forms.
Once your procedure is scheduled, you will receive a text message from 938-333-3995 or an email with a link and a pin to fill out your confidential pre-procedural health history. You will validate your log in with your date of birth.
If you have already received your pin through text message or email, you can also visit https://care.view2.ospitek.com with your pin, if you would prefer to complete this questionnaire on your computer or tablet.

Once you have completed, click Save and Finish to securely send us your information.
If you are not able to complete your health history online, please call our pre-op nurse during normal business hours at 614.232.6500 to complete your history over the phone.

Advanced Directives
The Center’s policy considers all patient’s eligible for life-sustaining emergency treatment such as resuscitation and transfer to a hospital in the rare event that their medical condition deteriorates.
If a patient should provide their advanced directive, a copy will be placed on the patient’s medical record and transferred with the patient should a hospital transfer be ordered by their physician.
At all times the patient or their representative will be able to obtain any information they need to give informed consent before any treatment or procedure.

Grievance
Polaris Surgery Center acknowledges that a patient may wish to file a formal grievance related to their care or other services. Grievances are taken very seriously and will go through a thorough investigation and ultimately formal review by a multi-disciplinary Grievance Committee.
You may file a grievance by:
1. Calling Polaris Surgery Center at (614) 232-6500.
2. Contacting the Ohio Department of Health by way of its Ambulatory Surgical Facilities department at 614-995-7466, by writing to them at: Bureau of Regulatory Operations, Ambulatory Surgical Facilities 246 N. High Street, Columbus, Ohio 43215 or by emailing them at community@ODH.ohio.gov.
3. Contacting the Joint Commission at: complaint@jointcommission.org or by phone at: (800)994-6610.
4. Contacting the Medicare Ombudsman by way of its Complaint Hotline at 1-800-MEDICARE (633-4227) or Medicare Ombudsman
